Submission Instructions: |
Air Carriers should use this form to request the appropriate records from the National Driver Registry (NDR). Since requirements vary from state to state, the requestor should check with the state in which the hiring air is located to confirm, request and receive the proper NDR request form IF the 8060-13 is not being used by that state. Detailed instructions for completion of 8060-13 are attached to each copy of the form.

There are two situations where a notarized signature will be required from the applicant when completing this form: (1) When the request is NOT made in person, and (2) if the state, where the request is being made, requires a notarized signature. Rules and procedures will vary from state to state, so there is no consistency in the NDR request process. Notarization should be completed in box III(b) of form 8060-13.
