Submission Instructions: 

This form should be completed by the hiring Air Carrier in order to request and receive Air Carrier and Other Records from any previous employers(s) Please see the accompanying instructions attached to the form itself for details. This form must be completed, dated, and signed by the Air Carrier, as well as the airman, and mailed or faxed to all previous employers that utilized that applicant as a pilot within the previous 5 years from the date on his/her application for employment.
